New Patients. How to register with the Practice.You can register as a patient with the Practice by bringing in your medical card and handing it to one of reception staff. You will need to fill in your new contact details on the card. If you do not have your medical card, ask for a patient registration (GMS1) form and hand this completed to reception. You will be asked to fill in a new patient questionnaire for each member of your family wishing to join the Practice. There is a form for children (under 16) and a different one for adults. An appointment will be made for you to see one of the Practice Nurses for a new patient medical examination. You will be given a letter giving details of the examination and what you will need to bring with you. If you are currently on prescribed medication you will also need to see your GP Patients have the right to express a preference to receive the services of a particular doctor. If you wish to do this, you must let us know this in writing. Whilst the Practice will endeavor to comply, it might not always be possible to do so.
